Basically, this PR lets you submit with or without the clientside JS and get the same output on the spreadsheet.
We received many error:error's from people who went through the tutorial and never added clientside JS... so I figured we could improve the experience.
The only reason we -required- clientside JS was preserving the field order. Instead, this PR lets you optionally define that field order, and only sends emails if a to address is set. These edge cases covered now let you submit without any clientside JS (you also lose our email validation & thank-you div if you don't use the clientside JS though).
Additionally, I incorporated the work introduced by @goatandsheep in #101 to support multiselects coming from just POST (no clientside processing) form data. I tested it with our sample form, and the output is now identical between our processed request & a default POST request, at least in Chrome.
